Our Community Health Survey

Healthy Lee is excited to announce the launch of a vital initiative aimed at assessing, understanding, and addressing the health needs of our community and the related social drivers of health (SDOH).

Your involvement will significantly contribute to the success of this effort and Healthy Lee’s collaborative impact to improve the health and well-being of our community.

They are gathering input from our community through the community survey. This quantitative assessment will be supplemental to the current Community Health Needs Assessment underway for 2023. Lee Health asks for our community participation and support. They will share the data collected with us and the broader community at the end of the survey.
